Fast, easy, and cost effective services
I needed a doctor to complete paperwork for me to receive a reasonable accommodation under the ADA at work. I submitted the forms that I needed filled out, paid a deposit, scheduled a video appointment with the provider, completed a questionnaire prior to the appointment and then met with the provider. The provider I met with was very friendly and promptly completed the necessary paperwork after my appointment. I received the completed paperwork a few hours after the appointment. Being able to complete the entire process from the comfort of my own home was amazing. The entire process was very fast and very easy. I would highly recommend using mydrsnote.com for any of your administrative health care needs.

This app is amazing
This app is amazing! I have been trying to find a doctor to get into since April just to get my FMLA paperwork signed I came across this website and got it the next day! && it was on a Sunday the doctor was amazing and it was fast and easy! Got my paperwork back in 1 business day. I was worried Sedgwick would give my issues but they approved the FMLA PAPERWORK right away!

The compassion my doctor showed me…
The compassion my doctor showed me meant everything and helped calm my anxiety. I finally felt like I was heard. The patient assistants they have are truly a blessing because I know my stress and constant checking up must be annoying but they are always so kind in their responses and assure me I will be ok and taken care of. Truly an amazing establishment all around.

Overall easy experience
There was an initial hiccup with scheduling due to time zones, but the Doctor quickly realized it and contacted me. We met and reviewed all of my chronic conditions, and he discussed the next steps. By the end of the day, I had my leave paperwork and then submitted it to my company. Work approved the paperwork a few days later and now I can feel secure when I am sick.

The real deal
I scheduled a visit with Dr. Sartin to get my paperwork certified for FMLA/STD after having surgery. They ask you a few questions in the beginning and ask for any paperwork you have up front so they can work on it during and after the appt. If you have any issues the team on the messaging platform responds almost instantly and it’s a real human. The price was exactly as stated and didn’t try to sell me on anything additional just truly listened to everything I said and then guided me through all my options. For the first time I felt like this was not only a breeze but a doctor actually listened to me and gave me all my options plus more if things didn’t get better in the 6 weeks. Well worth the money (especially using FSA/HSA) just for a peace of mind and not having to argue with someone. Dr Sartin also filled out another piece of paperwork my company asked for in less than a week after our initial appointment for no additional charge. If you’re on the fence about it I can assure you it’s well worth it to go for it because it will be the easiest thing you’ve ever done.

My primary care physician approved my disability leave but was slow to complete the required paperwork, and when it was completed, it lacked sufficient detail. As a result, I sought assistance from Dr. Sartin, who provided immediate and invaluable support. Dr. Sartin and his team are highly knowledgeable about the specific terminology and documentation required for claims approval.Dr. Sartin has assisted me with my Short-Term Disability paperwork, FMLA, ADA accommodations, and Washington Paid Family and Medical Leave. Without the support of Dr. Sartin and his team, I would likely have given up. I cannot adequately express how beneficial this service has been for me. In addition to their expertise, their understanding and personable approach made an incredibly difficult process much more manageable. I am deeply grateful for this service.
